The ECHO model addresses the gap in provider knowledge by linking academic medical center specialists with community-based primary care providers in case-based learning. The goal of this activity is to create a robust, engaged network of primary care providers and aging specialists who share resources and education, thus increasing provider capacity to care for older adults to improve health across Chicago.
