Target Audience
This activity is designed for primary care clinicians and other healthcare professionals dedicated to the care and treatment of geriatric patients.
Learning Objectives
After this activity, participants will be able to:
- Recall appropriate geriatric assessments in primary care;
- Identify local community resources that provide support relating to aging and health for patients and their families;
- Distinguish between typical signs of aging and those symptoms that may be associated with other health problems;
- List signs of dementia in geriatric patients;
- State the basic practices of palliative and hospice care;
- Describe advanced care planning, including important topics such as power of attorney and living will;
- Discuss polypharmacy & medication reconciliation for older adults;
- Generate a treatment plan for the management of dementia in geriatric patients.
