Community Dementia Resources and Best Practices for Integrating Into Your Work

Caring for individuals with dementia takes more than clinical expertise; it takes a strong network of community support. In this module, you'll explore how resources like transportation services, case management, meal delivery, exercise programs, and social engagement can greatly improve quality of life for patients and ease the burden on families, especially as the disease progresses.
 
You’ll learn how to stay informed about local services, overcome barriers such as geography or language, and help connect patients with the support they need. The module also covers essential planning steps, medical, financial, and legal, that help preserve patient autonomy and reduce stress on caregivers. Understanding power of attorney, state-specific elder rights, and when to involve legal support can make a meaningful difference in care.
 
Finally, you'll learn to recognize signs of elder abuse, neglect, and exploitation, risks that increase with dementia, and how to act quickly and effectively by working with Adult Protective Services and other safety networks. By building these skills, you’ll help ensure that people living with dementia receive care that is not only effective but compassionate, respectful, and safe.

Target Audience

This activity is designed for interdisciplinary healthcare providers, including nurses, nurse practitioners, physicians, physician assistants, psychologists, and social workers involved in the care of people aging with HIV and at risk for or living with dementia.

Learning Objectives

After this activity, the participants will be able to:

  • Identify legal tools that support autonomy and protection for older adults with dementia, including advance directives and powers of attorney.
  • Describe key community-based dementia resources and services, including those provided by AAA, the Alzheimer’s Association, and AARP.
  • Assess situations involving potential elder abuse to decide when referral to Adult Protective Services is appropriate.
  • Differentiate between home-based and facility-based dementia care options available in your region.
