Advance Care Planning and Transitions of Care
Advance care planning (ACP) is a powerful way to support dignity, comfort, and personal choice, especially for people living with dementia. While often overlooked in HIV care, ACP offers an opportunity to ensure patients’ voices are heard and respected throughout their care journey.
In this module, you’ll gain the tools and confidence to start meaningful conversations about future care preferences, including how to help patients appoint a medical power of attorney and complete medical orders for life-sustaining treatment. You’ll also learn how different team members can contribute, whether it’s HIV program staff introducing ACP or clinicians finalizing medical orders, and how to ensure that important documents are easy to access when needed.
By approaching ACP early and returning to it as needs change, you’ll help patients and families feel more prepared, supported, and aligned, leading to care that reflects what matters most.

Target Audience
This activity is designed for interdisciplinary healthcare providers, including nurses, nurse practitioners, physicians, physician assistants, psychologists, and social workers involved in the care of people aging with HIV and at risk for or living with dementia.
Learning Objectives
After this activity, the participants will be able to:
- Describe the purpose and components of advance care planning for people living with dementia.
- Assess client values and goals to guide advance care planning discussions using person-centered communication.
- Explain the roles of hospice and palliative care in supporting patients with advanced dementia.
- Analyze common barriers to effective care transitions and strategies to address them across the dementia trajectory.
