Advance Your Approach to Complex Diabetes Care!

Caring for patients with diabetes is increasingly complex, especially when medical, social, and behavioral challenges intersect. This practical, case-based series is designed specifically for community-based primary care clinicians who want real-world strategies they can apply immediately.

Through focused, evidence-based sessions, you’ll gain the tools, confidence, and clinical insight to implement and sustain patient-centered diabetes management in everyday practice. Learn how to navigate medically and socially complex cases, overcome common barriers to care, and deliver more coordinated, effective treatment that improves outcomes where it matters most.

Walk away with actionable skills, proven approaches, and solutions you can use the very next day. Join us to strengthen your practice and make a lasting difference for patients living with diabetes.
