These conditions remain a public health concern and lead to increased utilization and healthcare spending. Some of the primary goals of the national initiative focused on addressing MCC (Multiple Chronic Conditions) in the United States include strengthening health care and public health systems to improve gaps in effective treatment and management on an point of care as well as systems based level.

This activity will support one of the goals of the Department of Health and Human Services in providing primary care providers better tools and resources to improve care of individuals with chronic medical conditions.
