Update CME - Internal Medicine and Primary Care on Demand

Most recent estimates show that over 25% of US adults have multiple chronic health conditions and approximately 50% of adults suffered from at least one of several highlighted chronic conditions. These conditions remain a public health concern and lead to increased utilization and healthcare spending. Some of the primary goals of the national initiative focused on addressing MCC (Multiple Chronic Conditions) in the United States include strengthening health care and public health systems to improve gaps in effective treatment and management on a point-of-care as well as systems-based level.
