This summit provides a unique opportunity to collaborate on pressing health challenges identified in UChicago Medicine’s 2021-2022 Community Health Needs Assessment. Our focus will be on key priorities such as chronic disease prevention and management, building trauma resilience, and addressing inequities stemming from social determinants of health. This year’s summit will offer valuable insights through expert-led breakout sessions on these critical areas, an important update from the Urban Health Initiative (UHI), and a keynote presentation highlighting UHI's long-term impact and sustainability in the community.

Target Audience
This activity is designed for physicians, healthcare professionals, school representatives, and community-based organizations dedicated to identifying and treating asthma.
Learning Objectives
After this activity, participants will be able to:
- Recall the impact of the Urban Health Initiative (UHI) over the past decade and its future vision;
- Identify recent advancements in maternal and child health within the area and the role of community organizations;
- Summarize UCM’s Diversity, Equity, and Inclusion (DEI) initiatives and their impact on reducing health disparities;
- Outline the components of the Violence Recovery Program and its role in the community violence ecosystem;
- Describe the key components of pediatric asthma care from a community caregiver perspective;
- Examine the collaboration between patient advocates and the South Side Healthcare Collaborative (SSHCO) to reduce health inequities;
- Explain the role of community health workers and health ministry in improving community health outcomes;
- Assess the value of UHI's contributions to the South Side community and its long-term sustainability.
