Thromboembolic risk stratification is the key to efficient in implementation of the mechanical and pharmacological VTE prevention tools. While many thromboprophylaxis options are available and safe, defining the patient at risk is growingly clearer.  In the surgical population, the Caprini risk score has extensive validation and has recently been adapted so that patients can self-report their perioperative risk. Patients with a high Caprini score have a larger reduction in VTE rates when pharmacological prophylaxis is implemented. In the medically ill population, although crowning of the best tool still needs clarification, the IMPROVE score is closer to implementation. With modest success, this score was recently used as the main stratification tool in the largest trial to date for thromboprophylaxis in the medically ill population.

  • To discriminate the strengths VTE risk systems best validated in the surgical and non surgical hospital population;
  • To compare the impact of VTE in a global versus local perspective;
  • To formulate a risk and evidence-based action plan for VTE prevention in the hospitalized patients.
