Getting My Patient to Transplant: A View from the Trenches

The factors affecting transplantation referral are complex. We seek to bridge the gap between transplant listing and readiness for transplant so that over 50% of patients in a dialysis unit are ready for listing and are also ready for transplant at the time they are called to receive an organ. This half-day conference will give practitioners caring for dialysis and chronic kidney disease (CKD) patients insight and strategies about how to successfully navigate the transplant system. Patients with CKD and kidney failure (ESRD) have many co-morbidities that often make it difficult to get listed for kidney transplantation. Our conference is designed to explore these challenges and showcase the many resources that are available to assist. The conference is designed to be both didactic and interactive. The most common barriers to transplant are obesity and frailty. Presenters will inform participants about the resources available to overcome these barriers. Case presentations will allow discussion of the real challenges facing patients, and conference faculty will be able to problem-solve with participants.

Target Audience

This activity is designed for physicians, nurses, social workers, dieticians, and other healthcare professionals dedicated to providing superior care to dialysis and chronic kidney disease patients.

Learning Objectives

At the conclusion of this activity, participants will be able to:

  • Identify the challenges facing patients who wish to undergo kidney transplant;
  • Discuss services available to maximize the outcomes for patients and keep them ready for transplant;
  • Outline the biological factors that make physical characteristics such as frailty and obesity barriers to transplant;
  • Name psychosocial and economic challenges transplant patients face;
  • Evaluate different techniques to communicate effectively with transplant patients.
