Oak Street Health Advanced Care Planning Training

August 11, 2022 to August 18, 2022
This training will provide strategies for providers to have advanced care planning conversations with patients. Advance care planning conversations are the first step to completing advance directives and making important care decisions.

Target Audience

This activity is designed for Oak Street Health clinicians, social workers, and practice managers dedicated to the high-quality care of geriatric patients through excellent advanced care planning.

Learning Objectives

After this activity, participants will be able to:

  • Discuss strategies to incorporate advanced care planning knowledge into daily practice;
  • State how to apply informed communication skills and principles with patients and their families regarding advanced care planning;
  • Describe how to create and apply effective strategies for supporting clinic providers and staff in completing advanced care planning with patients;
  • Identify patients who are appropriate candidates for palliative and hospice care services;
  • Explain how to utilize appropriate legal documents to educate and support patients in advanced care planning completion.
