Advance Care Planning and Supportive Care Workshop

With its stated objective to address the health needs of older adults in Chicago through training healthcare professionals and Social Workers (MSWs) who treat geriatric patients, the SHARE (Southside Healthy Aging Resource Experts) Network is collaborating with Oak Street Health (OSH) to increase the capacity of OSH MSWs and Community Health Workers (CHWs) to engage patients in quality advance care planning and other topics related to caring for seriously ill patients. The current intervention will be a one-time only 2-hour training session that targets MSWs and CHWs to build competencies in goals-of-care discussions, advance care planning, and referral to supportive care for seriously ill patients, especially those approaching end-of-life. By equipping MSWs and CHWs with skills to engage patients in these discussions, the intervention will improve MSWs' and CHWs' understanding of patient goals, values, and wishes and align the care provided with what matters most to their patients. Furthermore, the ability to engage in difficult conversations will support ongoing quality care throughout the patient’s life cycle and increase referrals to palliative care and hospice services, programs underutilized by many marginalized and underserved communities.
Target Audience
This activity is designed for Oak Street Health clinicians, dedicated to high-quality care of geriatric patients throughout the patient’s life.
Learning Objectives
After this activity, participants will be able to:
- Recall how supportive care (palliative and hospice) can be essential for enhancing goal-concordant care, improving quality of life, and mitigating preventable acute care admissions for seriously ill patients;
- Identify appropriate candidates for palliative or hospice care services and articulate recommendations to primary care providers;
- Describe the range of services offered by palliative care and hospice for seriously ill patients and their caregivers.
