In this activity, participants will cover topics concerning strategies for team-based care and anemia.
Target Audience
This activity has been designed for physicians in internal medicine and family medicine, residents & fellows, nurses, APN's and other healthcare professionals involved in primary care.

Topic 1
Upon completion of this educational activity, participants will be able to:
- Become familiar the AMA Steps Forward Strategies for team-based care;
- Review and understand the Bellin Health model of team-based care;
- Appreciate and understand the benefits of team-based care on quality, patient loyalty and provider satisfaction.
Topic 2
Upon completion of this educational activity, participants will be able to:
- Develop an approach to evaluating the anemic patient in a primary care setting;
- Understand and review new forms of anemia recognized by the WHO and their prognosis;
- To make a decision on the appropriate treatment of the anemic patient in given clinical situations.
