This activity provides evidence-based updates and best practices across key areas of community medicine, with a focus on improving clinical decision-making, care coordination, and referral efficiency. By highlighting innovations, system capabilities, and referral pathways within a large academic-community health network, the program aims to enhance patient outcomes and strengthen integration across care settings.

Target Audience
This activity is designed for physicians and advanced practice providers from the UChicago Medicine Medical Group and faculty physicians from the Biological Sciences Division.
Learning Objectives
After this activity, participants will be able to:
- Explain the system-level factors influencing quality performance in primary care, with a focus on population health and timely diagnosis;
- Describe the structural evolution and strategic priorities of the UChicago Medicine Medical Group to support coordinated, system-wide care;
- Apply evidence-based diagnostic and management strategies for complex cardiovascular and metabolic liver diseases;
- Analyze surgical innovations in breast cancer treatment and abdominal aortic aneurysm repair to optimize patient referral and care planning;
- Evaluate emerging therapies for asthma, food allergy, and celiac disease to enhance personalized treatment plans.
