Update CME - Internal Medicine and Primary Care - CHI

Most recent estimates show that over 25% of US adults have multiple chromic health conditions and approximately 50% of adults suffered from at least one of several highlighted chronic conditions. These conditions remain a public health concern and lead to increased utilization and healthcare spending. Some of the primary goals of the national initiative focused on addressing MCC (Multiple Chronic Conditions) in the United States include strengthening health care and public health systems to improve gaps in effective treatment and management on an point of care as well as systems based level.
Constantly updated guidelines on treatment and management of these conditions necessitate primary care providers to continuously update their education from different knowledge sources. Interaction with human experts specifically for complex medical questions has been cited by physicians as a more effective mode of knowledge transfer. This activity will support one of the goals of the Department of Health and Human Services in providing primary care providers better tools and resources to improve care of individuals with chronic medical conditions.

Target Audience
This activity has been designed for physicians and other healthcare providers, primarily primary care providers in the internal and family medicine specialties.
Learning Objectives
At the conclusion of this activity, the learner will be able to:
- Primary care providers will be able to apply the most current treatment strategies to common chronic medical conditions provided in recent clinical guideline updates.
- Primary care providers will recognize effective interventions leading to reduction adverse effects of existing chronic conditions and prevention of additional comorbidities.
- Primary care providers will employ practice relevant new research in their everyday patient encounters to improve mortality and morbidity of common chronic medical conditions.
