Third Annual NorthShore and University of Chicago Hospice and Palliative Care Symposium
The overall goal of this program is to educate the attendee on how one can improve the quality of modern cardiac care through the use of the multi-disciplines found in supportive care programs like palliative care and hospice, especially in light of new technological advances that challenge the integrity of personal end-of-life values.
Target Audience
This activity has been designed for Cardiologists, Palliative Medicine, Primary Care Providers, Intensivists, Nurse Practitioners, Nurses, Social Workers, Chaplains, Bereavement counselors, Hospital Administrators and others interested in hospice and palliative care.
Learning Objectives
At the conclusion of the activity, participants will be able to:
- Evaluate the collaborative model of Palliative Medicine and Cardiology in heart failure management;
- Describe the advancements of device therapies for heart failure and how they impact end of life treatment decisions;
- Evaluate the psychosocial and spiritual impact device therapies have on heart failure patients and their families;
- Design a supportive care plan for severely ill heart patients that incorporate Advance Care Plans.
