How are we protected against GERD?
The LES provides a pressure barrier between the esophagus and stomach. It remains actively closed to prevent reflux of gastric contents
It is a physiologic region of increased or basal pressure
Relaxation is mediated by inhibitory neurons and occursa after swallowing to allow for food entry
The LES is strengthened by gastrin and weakened by cholecystokinin, secretin, and glucagon
Reflux occurs when the esophageal sphincter transiently relaxes, or when it doesn’t adapt properly to abdominal pressure
