Objectives:

• Understand the process of medication adherence and errors during transitions of care between the hospital and a skilled nursing facility.
• Review data on medication errors during transition of care to a skilled nursing facility.
• Implement and study an intervention on nursing education to reduce medication errors with admission to a skilled nursing facility.
• Determine future interventions to decrease medication errors with transitions of care.
