Objectives:

1.Review critically provision of care in the PICU for patients that died, suffered cardiopulmonary arrest, or acquired a nosocomial infection or line thrombus;

2. Identify strategies for improvement in patient care and safety, which can include creation of new protocols or management guidelines.
