1. Patient history and pertinent data review
2. Photo presentation (if applicable)
3. Evaluation, concerns, questions and comments presented by each member of the multidisciplinary team present
4. Team informed and updated on new or significant information
5. Case discussed and ongoing treatment and/or discharge plan formulated
